Rostered Leaders Forms to Bishop

To complete the Annual Rostered Leader's Report to the Bishop:

1. Be certain that the latest version of Formatta Filler is installed on your computer.  If you don't have Formatta Filler, download the latest version from the table below.

2. Click the link in the table below. This will take you to a page on the ELCA web site.  Then click on the appropriate form link, matching your roster category (lay or clergy) and your roster status (active under call from a congregation; active under call from the Synod Council or the Church Council; not currently under call [on leave, retired, or on continuing disability]).

3. Fill in the information requested.

4. There are two options for returning the form to the bishop.

   1. Send it to the synod as an e-mail attachment to synod@gulfcoastsynod.org.

   2. Print a copy and mail it (United States Postal Service) to the synod office at: 12707 North Freeway, Suite 580, Houston, TX  77060

Mobility Forms - Clergy

There are several steps to filling out your mobility paperwork.

1. First download the free Formatta Filler program.  There are two versions - Full Formatta Filler (3.17 MB - includes spell checker and dictionary) and Formatta Filler (1.6 MB - for those with slower connections). 

2. Double click the file to install. (Note: When the registration page for Formatta Filler opens, just close it.  You do not need to register it.)

3. Next, d ownload the file Ordained Mobility Form. 

It is strongly suggested that you review the forms after completing them. It is very easy to accidentally change the information. Click the lock button and enter a password to prevent others from making changes.

Attach completed forms to an email and send it to the synod office at synod@gulfcoastsynod.org  The form also can be printed and mailed to the synod office:

Mobility Forms - Associates in Ministry, Deaconesses, and Diaconal Ministers
